Start by filling out the contact form below. This notifies the committee of your interest and adds your name to the candidate pool.

Next, print and fill out the full application form. You’ll be asked to include details about your athletic achievements and background.

Turn in your completed form to your school’s athletic director. They’ll review all submissions and select the top candidates to represent your school.

Each school’s nominations are sent to the Old Grad Committee. Final winners are chosen and announced at the end of the school year.
